What are the deductibles for PrimeStar® dental plans?
Ameritas plans have no deductible for Preventive services, and a $50 deductible per person for Basic and Major services.
What are the benefit maximums? Are they per person?
PrimeStar plans have the option of a $1,000 or $2,000 benefit year maximum. This is per person. That means if more than
one person is on the policy (such as a husband and wife) they would each have their own benefit maximum.
Will my clients still be covered if they visit out-of-network dentists?
Yes. With an Ameritas plan, clients can visit any provider!
However, when they visit a network provider, they can enjoy an average discount of 25-50%. Our network is one of the nation’s

What vision providers are in-network?
PrimeStar Select grants access to the EyeMed network, which includes providers like LensCrafters, Pearle Vision, Target Optical,
